Triterpenoid saponins have significant structural and bioactive diversity. They are increasingly important in cancer treatment because of their efficacy and safety. In this chapter, the origin, pharmacological action, structure-activity relationship and clinical research of triterpenoid saponins against cancer have been updated, especially the molecular mechanism of its therapeutic properties. Relevant literature and research reports were collected through PubMed. Anti-cancer triterpenoid saponins by regulation and cell proliferation, apoptosis, autophagy, transfers, angiogenesis, inflammation, oxidative stress, multi-drug resistance, cancer stem cells and the multiple signals associated microRNAs targets to inhibit the formation and development of cancer. The molecular basis of triterpenoid saponins in tumor chemotherapy and chemotherapy was reviewed.
